EXPERIENCES WITH VARIOUS FASCIOCUTANEOUS TERRITORY FLAPS

Abstract
|

|
Fasciocutaneous flap is a new type of local flap which includes the skin, the subcutaneous fat and the fascia but not the muscle.
Three different arterial systems supplying the deep fascia are perforating arteries, subcutaneous arteries, subfascial arteries and together are responsible for the excellent collateral and the good perfusion of the deep fascia.
The vascular pattern of a flap on the lower leg that included the deep fascia is much denser than that in a similar flap without fascia.
Fasciocutaneous flap is very useful in the repair of soft tissue defects on the lower leg, and inclusion of the deep fascia in flaps designed in various areas of the body appears to confer an element of safety, so that the surgeon need not restrict himself or herself to the usual 1:1 ratio.
Fasciocutaneous flaps have several advantages and summerized as follows:
1. It is easy to design and construct large flaps that are safe because of the good circulation.
2. The operating time is short for an experienced surgeon.
3. It is possible to carry out surgery on the underlying bones at the same time.
4. The postoperative treatment is simple.
5. There is very little bleeding from the dissected surfaces of these flaps.
This report describes the use of various fasciocutaneous territory flaps which are inferior gluteal thigh flap,tensor fascia lata flap,fasciocutaneous transposition flap, reverse radial forearm flap and turn over reverse dermal flap.
All cases were successfully treated without significant complication.
